Resources for Our New St. Cloud Area Dental Patients
At Friesz Family Dentistry, we make the registration process seamless and easy with online patient information forms and our experienced staff ready to answer your questions and address concerns.
Forms and Documents to Bring
We provide medical history update, registration and privacy forms online for both new and returning patients to view, print, fill out and bring into the office, expediting the registration and information update process. Please fill out all fields on our forms so we can provide proper diagnosis and treatment.
Although your form may completely list your insurance coverage, we request that you bring in your insurance coverage card along with proof of ID on your first visit so we can verify your coverage for you or your dependents. Changes in your insurance coverage require that you notify us ahead of your next appointment and provide us with your new insurance card on your next visit to verify your change in coverage for you or your dependents.
We value the confidentiality of your patient information and advise that new patients bring their registration forms in with them; sending your health information by email may put your information at risk of being intercepted. Friesz Family Dentistry strictly adheres to HIPAA Law, and actively works to process and securely store your information according to health care laws. With Friesz Family Dentistry, we value your personal information and want to carefully handle your private documents from unsolicited outside sources.
As a patient with us you have access to your medical records. Simply fill out a medical records request form and a member of our staff will process your request and release a physical copy of your information to you. Please make sure to secure your physical copies of your information in a safe place, or safely dispose of your records should you no longer need them. Our staff will gladly and safely dispose of your extra copy of your medical records should you want or need to. We believe in empowering our patients to be active participants in their dental health care; don’t hesitate to contact us if you have any questions about our policies or protocols. Our staff will answer any questions or concerns you have.
